Application Instructions

• An SSL certificate encrypts and protects your data
• Use mixed case for your all fields. For example:
  My Company (correct)
  my company (incorrect),
  MY COMPANY INC (incorrect)
• DBA:
  • Fill this in only if you invoice using a trade style rather than your corporate or LLC legal name. The name will appear on your invoices. E.g., High Pacific, Inc dba Surf Pants.
  • Proprietorships: Enter Proprietorship name in "Company" field and leave DBA field empty.
• Address1: P.O. Box address are unacceptable. Fill in the company’s street address.
• Address2: Enter suite numbers, unit numbers, etc. here.
• Secretary of State Info:
  • Please use this link if you are unsure. Enter "N/A" (no quotes) if your company was chartered in New York state, because it does not provide ID numbers.
  • Proprietorships: leave blank
• Organization Type: Must be absolutely correct or you will have to start over.
• US Govt/Louisiana contracts: Only check these buttons if you have contracted directly with an agency of the US Govt or the State of Louisiana. Don’t do so if your contract is with another company that has such a contract (i.e., general contractor).
